Eli Lilly has reported encouraging early results for a new JAK inhibitor targeting myelofibrosis, a rare and difficult-to-treat bone marrow cancer that disrupts normal blood cell production.

The drug came to Lilly through its acquisition of Ajax, a biotech company. According to BioPharma Dive, the treatment showed positive effects in patients with hard-to-treat forms of the disease — a patient population that typically has few good options.

The results caught the attention of analysts, who suggested the drug could eventually pose competition to Incyte's Jakafi, currently one of the most established treatments in the myelofibrosis space.

JAK inhibitors work by blocking a group of enzymes involved in inflammatory signaling that drives the disease. Myelofibrosis causes scarring in the bone marrow, leaving patients with fatigue, enlarged spleens, and a shortened life expectancy. Existing treatments help manage symptoms but rarely cure the disease, making new entrants in this class closely watched.

If Lilly's drug continues to show strong results in later-stage trials, it could give patients and doctors a new option — and put meaningful pressure on Incyte's dominance in a market where competition has historically been limited.
